Drug Testing Consent Form

I have applied for employment with Star Electric. As a condition for my application being considered, I understand and agree to undergo substance screening. I understand that if my test results are positive, I shall not be considered further by Star Electric. If for any reason (except layoff) I leave the employment of Star Electric during the first 90 days after hired; I understand that the cost of the Drug Test will be withheld from my final paycheck.

I hereby authorize any physician, laboratory, hospital or medical professional retained by Star Electric for screening purposes to conduct such screening and to provide the results to Star Electric. I also release Star Electric and any person affiliated with Star Electric and any such institution or person conducting the screening, from liability therefore.

Star Electric Company of Texas (“Company”) sends text messages to employees at the phone number provided during the hiring process.
